Charlene Hayzlett

Phyllis Charlene Hayzlett was born May 20, 1932 in Forbes, Missouri, to Charles and Vivian (Brock) Polaski. Charlene graduated from Forbes High School.

On June 10, 1951 she married C. Melvin Hayzlett in Oregon, and they became parents of a son and a daughter.

Charlene passed away on Thursday September 8, 2016 at the Oregon Care Center.

She was preceded in death by her parents; husband, Melvin; and brother, J. Richard Polaski.

Survivors include her son and daughter-in-law, Randy and Hope Hayzlett of Oregon; daughter and son-in-law, Sharon and Eldon Fansher of Forest City; sister, Mary Lee (Albert) Munyon; five grandchildren and their spouses, Christopher and Lisa Fansher, Craig and Amanda Fansher, Samantha and Douglas Suntrup, Stacey and Dennis Law, and Glenna and Ryan Crews; 11 great grandchildren, Haleigh and Justin Law, Candence and Jackson Crews, Dawson and Dane Fansher, Sydney Markt, Jacelyn and Colton Fansher, Dylan Schultz and Braxton Suntrup ; and nieces and nephews.

Services: Sunday, Sept. 11, 2016, 2:30 pm, Oregon United Methodist Church. Family will greet friends one hour prior to services, at the church. Interment: Maple Grove Cemetery. Memorials: Oregon United Methodist Church or the Holt County Cancer Fund.
